Stepping Forward is passionate about equipping and supporting those that want to break free from the constant struggle against chronic disease and are in need of supportive services. We provide empowering education, case management services and transformational life coaching. Stepping Forward is here to come alongside you to bring about the changes to move you forward and improve your quality of life!

Our focus is on these four core areas-
1. Diabetes Prevention and Management

•  in a virtual, small group setting
• Providing 1:1 online support for those with more challenging issues

2. Patient Advocacy & Case Management

• Assisting with navigating health care issues and various medical needs (referrals, medical appointments, transportation, communication with Providers, medication management, equipment, hospitalizations, and transition of care)
• Providing community resources and services to enhance quality of life and wellness needs. Includes; Mental, Emotional, Spiritual, Financial, Physical, Environment, Occupational, and Social wellness

3. Healthcare Education for Professionals 

• Providing disease management workshops for healthcare professionals and those serving in Independent Living, Assisted Living, Adult Care Homes, and similar facilities. Call for our 'Favorites' list of classes or to create something just for you. (CE's may be available)

4. Life Coaching

• We provide Life Coaching in groups and  for those that need individual coaching. Our focus is to assist you in discovering & accomplishing the plans and goals that you desire! We will provide guidance and accountability to help you gain clarity on the vision you want out of life.  We offer support and resources, as we facilitate the direction on what you ultimately want to accomplish and desire for yourself!
